All you have to do is just go to the hospital and you'll see Jane doe, when you do her quest you need 20 pieces of a broken mirror you can find from wild boar near Perion then after u just need 4 charms from zombie mushroom and bring it to her then you'll get the cape.

She lowered the charms down to 4 instead of 100 which makes it easier

Where can u upgrade it?

Its Jane Does quest. you need to collect 20 broken mirrors from wild boars then take it to her and then get 100 charms of the undead and take those to her and she gives you the cape
